How do no-kill shelters work?

If I understand correctly no-kill shelters keep animals alive indefinitely until they are adopted. Does that mean the animal could potentially spend the rest of it's life in a cage if no one adopts it (cute over ugly and all that)? To me that sounds far crueler than euthanasia.
