Do I have the symptoms for Breast Cancer (Paget's Disease of the Nipple). Do I need to go to my doctor ASAP?

I'm 20 years old and for the past month I've been experiencing nipple pain. For the past 3 weeks I've been experiencing very itchy, dry on both . It sometimes looks a bit scaley. The symptoms come and go. My nipple also is flattened against my sometimes. I'm not sure if it's normal... I tried to see if I can feel any lumps but I wasn't sure how it's supposed to feel. My always feels lumpy.
